Camp Jack Volunteer Form
I wish to volunteer at Camp Jack and make a commitment to give a little of my time to helping Veterans of the U.S. Armed Services and their families. I understand that I am a volunteer and serve without compensation except as allowed in the Constitution and By-Laws.
The object and purpose of Camp Jack is to assist local Veterans of the U.S. Armed Services and their families with respect, compassion and honor and to help improve their quality of life.

Release: I hereby assume all risk of injury or harm as a result of the activities specified above and agree to release, indemnify, defend, and forever discharge Camp Jack from all liability, claims, demands, damages, costs, expenses, and causes of action due to death, injury, loss, or damage to the undersigned.
