Camp Jack Addendum to Volunteer Form

As of November 8, 2021, Camp Jack is required by our insurance carrier to obtain signatures on several documents that will become part of your volunteer file.   We request that you read, sign and submit the Attestation Statement, Non-Disclosure Agreement and the consent to background check (shown below), to Camp Jack either in person to our office, or you can scan the signed documents and send them via email to: campjack@info.com or you can complete the forms on-line and send by clicking on the "Submit" button.
